Waco Family Medicine is constantly working on providing patients with better health care service. We are the first health organization in Waco to receive Patient Centered Medical Home recognition. Ten of our clinical sites have been granted full PCMH recognition Level 3 (the highest level possible).
Are you a new Patient?
For your convenience, we have put together important documents that are needed to complete your initial registration. Please print and complete the forms in the Welcome Packet and bring them with you to your scheduled appointment. This will speed up your registration process. Also, be sure to bring your current insurance card along with your immunization record. If you do not have medical coverage we suggest you contact the Eligibility Department to help with financial assistance programs.
